Background: Spinocerebellar ataxias (SCAs) are a group of neurodegenerative genetic diseases characterized by movement disorders that can affect nutritional status and body composition. This study sought to assess body composition in SCA3 and SCA10 patients. Methods: Anthropometric assessments and bioelectric impedance analysis were performed in 46 SCA3 and SCA10 patients and 76 controls of both genders. Results: Of the patients, 69.6% had SCA3 and 58.7% were women. SCA3 patients had significantly lower percentages of body fat (%BF) than controls (15.0 ± 6.1 vs. 20.6 ± 7.1; p=0.014) and (22.4 ± 6.9 vs. 30.1 ± 6.0; p<0.001), respectively. Among the women, there was a statistically significant difference in %BF between SCA3 and SCA10 patients (22.4 ± 6.9 vs. 32.4 ± 4.9; p<0.001). Male and female SCA3 patients had significantly lower fat-free mass (FFM) than controls [50.6 kg (46.9-54.7) vs. 58.6 kg (52.6-63.9); p=0.001] and [38.2 kg (35.1-42.6) vs. 42.8 kg (39.7-46.1); p=0.004], respectively. Male SCA10 patients also had lower FFM than controls [51.2 kg (47.1-55.4) vs. (52.6-63.9); p=0.008]. Female SCA10 patients had significantly higher FFM than controls and SCA3 patients [45.0 kg (43.3-45.6) vs. 42.8 kg (39.7-46.1); p=0.004] and [45.0 kg (43.3-45.6) vs. 38.2 kg (35.1-42.6); p=0.004], respectively. There was moderate correlation (-0.42) between disease duration and muscle mass (MM), and weak (-0.38) between SARA (Scale for the Assessment and Rating of Ataxia) and MM in SCA3. In SCA10, there was no significant correlation between these variables. Conclusion: Female SCA3 patients had more body composition changes than female SCA10 patients, mainly in relation to FFM. SCA3 and SCA10 patients need nutritional follow-up to minimize body compartment changes.

INTRODUCTION: Cardiovascular diseases are the main cause of mortality and disability worldwide, so the prevention becomes a priority in terms of public health. Therefore, it is necessary to use validated strategies to adequately identify these patients in daily clinical practice. The objective of this scope review is to comprehend and comprehensively describe the anthropometric indicators used in studies, such as such as weight, height, circumferences, lengths and skin folds, that address its association with coronary artery calcification to identify cardiovascular risk in the adult population. METHODS AND ANALYSIS: Using Arksey and O'Malley's scoping review methodology as a guide, our scoping review of published reviews begins by searching several databases: Cochrane Central Register of Controlled Trials in the Cochrane Library, Medline Complete (EbscoHost), Embase, LILACS (Literatura Latino-Americana e do Caribe em Ciências da Saúde-BIREME Literatura Latino-Americana e do Caribe em Ciências da Saúde)and Web of Science and Scielo. As well as, it will be searched in the International Platform of the Registry of Clinical Trials of the WHO (www.who.int/ictrp); ClinicalTrials.gov; Transforming Research into Practice. Our team has formulated search strategies and two reviewers will independently screen eligible studies for final study selection. Bibliographic data and abstract content will be collected and analysed using a tool developed iteratively by the research team. ETHICS AND DISSEMINATION: This protocol reports a comprehensive, rigorous and transparent methodology. This scoping review will be the first study to compare anthropometric measurements and coronary artery calcification, and thereby will contribute to the design and comparison of future studies in this field. This protocol reports a comprehensive, rigorous and transparent methodology. The results will be disseminated through a peer-reviewed publication. By identifying gaps in the current body of literature, this study can guide future research.

Background Complex percutaneous coronary intervention (PCI) is associated with higher ischemic risk, which can be mitigated by long-term dual antiplatelet therapy (DAPT). However, concomitant high bleeding risk (HBR) may be present, making it unclear whether short- or long-term DAPT should be prioritized. Objectives This study investigated the effects of ischemic (by PCI complexity) and bleeding (by PRECISE-DAPT [PRE dicting bleeding Complications in patients undergoing stent Implantation and Sub sequent Dual Anti Platelet Therapy] score) risks on clinical outcomes and on the impact of DAPT duration after coronary stenting. Methods Complex PCI was defined as ≥3 stents implanted and/or ≥3 lesions treated, bifurcation stenting and/or stent length >60 mm, and/or chronic total occlusion revascularization. Ischemic and bleeding outcomes in high (≥25) or non-high (<25) PRECISE-DAPT strata were evaluated based on randomly allocated duration of DAPT. Results Among 14,963 patients from 8 randomized trials, 3,118 underwent complex PCI and experienced a higher rate of ischemic, but not bleeding, events. Long-term DAPT in non-HBR patients reduced ischemic events in both complex (absolute risk difference: −3.86%; 95% confidence interval: −7.71 to +0.06) and noncomplex PCI strata (absolute risk difference: −1.14%; 95% confidence interval: −2.26 to −0.02), but not among HBR patients, regardless of complex PCI features. The bleeding risk according to the Thrombolysis In Myocardial Infarction scale was increased by long-term DAPT only in HBR patients, regardless of PCI complexity. Conclusions Patients who underwent complex PCI had a higher risk of ischemic events, but benefitted from long-term DAPT only if HBR features were not present. These data suggested that when concordant, bleeding, more than ischemic risk, should inform decision-making on the duration of DAPT. (AU)

BACKGROUND AND OBJECTIVE: Recent studies have related the consumption of ultra-processed foods with the development of cardiovascular diseases and have considered this relation with excessive sodium intake. While Brazilian studies have analysed that this consumption may have no direct relationship with the processing, but rather with the addition of salt. The purpose of this study is to investigate the contribution of the consumption of processed products in the daily intake of sodium ingestion by atherosclerotic disease patients. METHODS: A sub study, conducted with data from 630 cardiopathic patients who take part in "Effect of Brazilian Cardioprotective Food Program study on the reduction of events and risk factors in secondary prevention of cardiovascular disease part". Food was classified as: unprocessed or minimally processed foods, processed culinary ingredients, processed foods, ultra-processed food and beverages. Twenty-four-hour food recall (R24h) was collected from patients, and the estimation of total calories and sodium intake were calculated, as well as the percentage of sodium contribution according to the categories already mentioned. For the adequacy ratio analysis, the daily values of sodium intake were used to compared to the recommendations of the World Health Organization (<2000 mg / day). RESULTS: The average sodium intake was 1970.87 mg for women and 2642.86 mg for men, being higher for males' patients aged 60-79. It was observed that 64% of the studied population demonstrated sodium intake > 2000 mg. When considered levels > 3001 mg, a higher incidence of consumption was observed in the male group. Only 21.1% were intaking sodium within the recommended amount. Industrialized foods contributed to 33% of the mineral intake. CONCLUSION: These findings have demonstrated that the majority of the studied patients exceeded the dietary sodium recommendation. It has also indicated that patient's male, have presented increased consumption of the mineral. Consequently, warning for the necessity of greater investments in the nutritional re-education of these patients.

Objective: to compare anthropometric equations with one that uses BIA to predict muscle mass in patients with SCA. Methods: 76 patients with ages from 22 to 72 years had their muscle mass estimated by the equation that employs BIA and by five anthropometric equations. The results were analyzed using Pearson's and intraclass correlation coefficient, the paired t test and the Bland-Altman plot. Results: the mean weight and height ± SD were 64.2 ± 14 kg and 1.61 m ± 8 cm, respectively. The equation proposed by Lee et al. (2000), which uses body weight and height measurements, produced the best results in predicting muscle mass, since a significant bias value was not detected, and both a stronger linear correlation (r = 0.94) and higher intraclass correlation (ICC = 0.93). Conclusion: this anthropometric equation can be used to reliably estimate and monitor decreases in muscle mass in people with SCA


Introducción: las ataxias espinocerebelosas (AEC) forman parte de un grupo de enfermedades que afectan al equilibrio y la marcha y al trofismo muscular. La utilización de la impedancia bioeléctrica (BIA) es cada vez más común en la evaluación de la composición corporal. Sin embargo, esta tecnología no siempre está disponible en los servicios de salud y su uso en un elevado número de personas presenta ciertas limitaciones. Objetivo: comparar diferentes ecuaciones antropométricas con una ecuación que utiliza valores de BIA en la predicción de la masa muscular en un grupo de pacientes con AEC. Métodos: se estimaron las cantidades de masa muscular de 76 pacientes portadores de la enfermedad con edades comprendidas entre los 22 y los 72 años con base en la ecuación que utiliza la BIA y otras cinco ecuaciones antropométricas. Para el análisis de los resultados se emplearon los coeficientes de correlación de Pearson, de correlación intraclase, el test de t pareado y el análisis de Bland-Altman. Resultados: la media de peso y altura ± desvío patrón fueron 64,2 ± 14 kg y 1,61 ± 8 m, respectivamente. La ecuación propuesta por Lee y cols. que utiliza parámetros de peso y estatura presentó mejor desempeño en la predicción de masa muscular pues no presentó sesgo significativo, mayor correlación lineal (r = 0,94) y mayor correlación intraclase (ICC = 0,93) en relación a las demás ecuaciones. Conclusiones: esa ecuación antropométrica se puede utilizar para estimar la masa muscular de los portadores de AEC

BACKGROUND: : Obesity is one of the main causes of glycemic change. Failure of clinical obesity treatment may lead to an increase in bariatric surgery. Dietary guidance, in conjunction with disabsorptive and hormonal factors resulting from the anatomical and physiological changes provoked by the surgery, is associated with changes in food intake. AIM: To analyze food intake evolution during the first postoperative year of Roux-en-y gastric bypass in patients with type 2 diabetes mellitus or glycemic alteration. METHODS: : This was a longitudinal and retrospective observational study. For food intake evolution analysis, linear regression models with normal errors were adjusted for each of the nutrients. RESULTS: At 12 months, all patients presented improvement in glycemic levels (p<0.05). During the first postoperative year, there was a reduction in energy intake, macronutrients, consumption of alcoholic beverages and soft drinks. Conversely, there was an increase in fiber intake and diet fractionation. It was observed that, despite gastric restrictions, the micronutrient intake specifically recommended for glycemic control was greater up to six months postoperatively. CONCLUSION: There was change in the quantity and quality of food intake. It was the most prevalent glycemic control contributor up to six months postoperatively. At the end of one year, the diet underwent a change, showing a similar tendency to the preoperative food intake pattern.

RESUMO Racional: Obesidade é uma das maiores causas de alteração glicêmica. O insucesso no seu tratamento clínico pode levar ao aumento de operações bariátricas. Orientação dietética, em conjunto com fatores disabsortivos e hormonais resultantes das alterações anatômicas e fisiológicas provocadas pela operação, está associada à mudanças na ingestão alimentar. Objetivo : Analisar a evolução da ingestão alimentar durante o primeiro ano pós-operatório de bypass gástrico em Y-de-Roux de pacientes com diabete melito tipo 2 ou alteração glicêmica no pré-operatório. Métodos : Estudo observacional longitudinal e retrospectivo. Para análise da evolução da ingestão alimentar, modelos de regressão linear com erros normais foram ajustados para cada nutriente. Resultados: Aos 12 meses, todos os pacientes apresentaram melhora nos níveis de glicemia (p<0,05). Durante o primeiro ano pós-operatório, houve redução na ingestão de energia, macronutrientes, consumo de bebidas alcoólicas e refrigerantes. Por outro lado, houve aumento na ingestão de fibras e fracionamento de dieta. Observou-se que, apesar das restrições gástricas, a ingestão de micronutrientes recomendados especificamente para o controle glicêmico foi maior até seis meses do pós-operatório. Conclusão: Houve mudança na quantidade e na qualidade da ingestão alimentar, sendo o consumo de alimentos que contribuem para o controle glicêmico mais prevalente até o sexto mês de pós-operatório. No pós-operatório em até um ano, a dieta sofre alteração, apresentando inadequações em relação à pirâmide específica, com tendência ao padrão alimentar do pré-operatório.
